In early 1967, NASA was preparing for the first crewed flight of the Apollo program, designated AS-204 and later designated Apollo 1. The mission was scheduled to launch in February 1967 as a low-Earth orbit test of the Block I Command and Service Module. However, the spacecraft was plagued by technical glitches, poor quality control, and schedule pressure driven by President John F. Kennedy's goal of reaching the Moon before the decade was out. The prime contractor, North American Aviation, had struggled with design flaws, complex wiring layouts, and flammable materials within the cabin.

On January 27, 1967, astronauts Virgil "Gus" Grissom, Edward H. White II, and Roger B. Chaffee stepped into Spacecraft 012 at Space Launch Complex 34 at Cape Kennedy for a routine "plugs-out" test. The capsule was pressurized with 100 percent pure oxygen at 16.7 pounds per square inch—higher than sea-level atmospheric pressure—to simulate spaceflight conditions. Throughout the day, the crew experienced persistent communications problems, leading Grissom to famously remark, "How are we going to get to the Moon if we can't talk between two or three buildings?"

At 6:31 p.m. EST, telemetry recorded a momentary voltage surge, likely caused by an electrical short circuit near the lower equipment bay. A spark instantly ignited flammable Velcro, nylon insulation, and plastic mesh inside the pure-oxygen environment. Within seconds, a violent flash fire swept through the cabin. The high internal pressure caused by the expanding hot gas prevented the astronauts from opening the complex, inward-opening hatch mechanism. Within fifteen seconds, the pressure hull ruptured, venting thick black smoke and flame onto the gantry level.

Support technicians braved dense smoke and intense heat to reach the crew, but by the time they forced open the hatch, all three astronauts had died of asphyxiation from carbon monoxide inhalation and thermal burns. The tragedy prompted a complete halt to crewed Apollo flights, triggering a massive congressional investigation and a thorough overhaul of NASA's safety culture, spacecraft design, and material selection standards.
